Must be Medicaid eligible, currently receiving or approved for personal assistance, expected to need services for at least 180 days, be self-directing or have someone who can be responsible for the program on your behalf.

NYS resident, who qualifies for Medicaid and is elderly, has a disability, or requires assistance with activities of daily living, or needs skilled care. Must be medically stable and capable of overseeing their own care or have a designated representative.
