Income must be at or below 250% of the federal poverty level, have no other resources available to pay including federal, state, or charity programs, and have not received New Eyes voucher in the last 12 months.

Be unable to read standard printed materials because of visual or physical disability (missing limb, severe arthritis, etc.) or have a diagnosed reading​/learning disability.
