ADULT: • Age 21 and older. • Medicaid recipient. • Two or more chronic conditions. Chronic conditions may include substance use disorder, asthma, diabetes, heart disease, being overweight, and others. HIV​/AIDS and serious mental illness also qualify. CHILDREN: • Birth to age 21. • Active Medicaid. • Two or more chronic conditions. Chronic conditions may include mental health, substance use disorder, asthma, diabetes, heart disease, being overweight, and others. HIV​/AIDS and having experienced significant trauma can also qualify.

Call for information. Forensic Case Management must be incarcerated in Ontario County.

Adults age 18 and over. Adults do not have to be referred from an agency or provider, self-referrals are accepted for all treatment programs. Adolescents age 13-17 by referral only.

Adult Medicaid recipients including managed care and dual-eligible Medicaid and Medicare. Receiving the majority of their care in Monroe County, and the following: Significant mental illness or serious emotional disturbance. HIV​/AIDS. Complex trauma, if under 21. Two or more chronic health conditions (i.e. asthma, diabetes, heart disease, mental health condition, substance use disorder, etc.).
