Families of children 21 years and under receiving medical treatment within area served. Westmoreland House: A referral from a health care provider or social worker, and must live farther 12 miles of the Westmoreland House. House within The Hospital: Children must be inpatient at Strong Memorial Hospital. Family member must be 18 or older to stay here. Family Rooms & Lounge: Open to families of pediatric patients. Mental Health and Wellness Family Room: Open to families of pediatric patients receiving care in the Department of Psychiatry at Strong. Unity Sleeping Rooms: Children must be inpatient at Unity Hospital. Family member must be 18 years or older to stay.

Must be Medicaid eligible​/active Medicaid recipient and have: 2 chronic health conditions (asthma, diabetes, heart disease, COPD, obesity, substance abuse disorder, etc.) OR A single qualifying diagnosis, including serious mental illness (Bipolar Disorder, Schizophrenia, etc.), HIV​/AIDS, and significant behavioral, medical, or social risk conditions that can be addressed through care management.
