Age 18 or older. Must meet the SMI criteria for each program and additional documentation is required to go with each referral. It is recommended that family, individuals, and or providers contact SPOA directly to discuss a possible referral. Call for information.

Eligibility:
None for mental health evaluation (referred via courts​/justice system). Medication assistance: individuals with mental illness being released from jail, local hospital, or individuals who return to Monroe County from a state prison and have applied for Medicaid.

Eligibility:
Diagnosis of psychotic illness. Application filled out by professional care provider. Young Adult Program: ages 18-25.
