People With Chronic Illnesses

Individuals who have serious medical conditions that have persisted over a long period of time or are subject to frequent recurrences, are treatable but rarely cured completely and require persistent self-management behaviors. Chronic illnesses may be life-long in duration and/or progressive in nature; and many are marked by sudden flare-ups that can be caused by stress or other factors. Medicine and other treatments can often help people with chronic illness lead a normal life, although many require long-term care.

Eligibility:
CHILDREN: Birth to age twenty-one, active Medicaid., two or more chronic conditions. ADULT: Age 21 and older. Medicaid recipient.Two or more chronic conditions. Chronic conditions may include mental health, substance use disorder, asthma, diabetes, heart disease, being overweight, and others. HIV​/AIDS and having experienced significant trauma can also qualify.

Individuals with a developmental disability and who have been approved for eligibility through OPWDD. In-home respite open to all ages. Individual must live with family member. Respite program for medically frail primarily for folks who have, or are close to, aging out of school services.
