Individual 18 or older, who receives Medicaid and has a chronic medical diagnosis.

Adults with active Medicaid who have two or more chronic conditions. Chronic conditions may include mental health, substance use disorder, asthma, diabetes, heart disease, being overweight, HIV​/AIDS and others.

Ages 18 or older. Must be a Medicaid recipient, must meet one of the following conditions: a significant mental illness two or more chronic health conditions such as asthma, diabetes, heart disease, mental health condition, or substance use disorder, must have risk factors that impact daily life functioning.

Eligible Medicaid members with chronic illnesses. Call for information.

CHILDREN: • Birth to age 21. • Active Medicaid. • Two or more chronic conditions. Chronic conditions may include mental health, substance use disorder, asthma, diabetes, heart disease, being overweight, and others. HIV​/AIDS and having experienced significant trauma can also qualify.

Individuals with a disability, seniors, live in the community in New York State, need Medicaid and have excess income or resources to shelter.

Birth to twenty one. Medicaid (or Medicaid Managed Care) recipient and live in area served.

Resident of area served, child​/youth under the age of 21, enrolled in Medicaid. The child​/youth must meet the NYS DOH eligibility. Significant behavioral, medical, or social risk factors which can be addressed through care management.

Ages 6-21, receiving Medicaid, with a chronic medical diagnosis.

Children from birth to age 21 and must have an active Medicaid AND two Chronic Conditions OR one of the following single Qualifying Conditions: -HIV​/AIDS -Serious Emotional Disturbance (SED) -Complex Trauma
