Programs that maintain lists of individuals who are available to serve as care/case managers and which link individuals who are in need of this service with appropriate resources. Care/case managers develop plans for the evaluation, treatment and/or care of individuals who, because of age, illness, disability or other difficulties, need assistance in planning and arranging for services; which assess the individual's needs; coordinate the delivery of needed services; ensure that services are obtained in accordance with the case plan; and follow up and monitor progress to ensure that services are having a beneficial impact on the individual.

Health Home Care Management

Thru: Bridges For Brain Injury, Inc

Eligibility:

In order to be eligible for Health Home services, the individual must: Be 21 years or older Enrolled in Medicaid Live within the following New York counties: Monroe, Ontario, Livingston, Wayne, Seneca and Yates And have one or more of the following conditions: Signiﬁcant mental illness or serious emotional disturbance HIV/AIDS Two or more chronic health conditions (i.e. asthma, diabetes, heart disease, mental health condition, substance use disorder, etc.)
