Children from birth to 21 years, must have two or more chronic conditions, or Significant Emotional Disturbance, or Complex trauma, or HIV​/AIDS. Must be enrolled in Medicaid.

Adult Medicaid recipients including managed care and dual-eligible – Medicaid and Medicare. Receiving the majority of their care in Monroe County, and the following: significant mental illness or serious emotional disturbance HIV​/AIDS complex trauma, if under 21 two or more chronic health conditions (i.e. asthma, diabetes, heart disease, mental health condition, substance use disorder, etc.

Can accommodate other languages as needed.

New York State resident. U.S. citizenship is not required. Eligible if waiting for Medicaid or insurance coverage. Eligible to teen-agers if unable to use their parents' money or resources. Call for income guidelines which is the same for all components of the program.
