Birth to 21 years of age. Must be enrolled in Medicaid and have 2 chronic health conditions (HIV​/AIDS, Serious Emotional Disturbance or Complex Trauma, and other disabilities).

ADULT: • Age 21 and older. • Medicaid recipient. • Two or more chronic conditions. Chronic conditions may include substance use disorder, asthma, diabetes, heart disease, being overweight, and others. HIV​/AIDS and serious mental illness also qualify. CHILDREN: • Birth to age 21. • Active Medicaid. • Two or more chronic conditions. Chronic conditions may include mental health, substance use disorder, asthma, diabetes, heart disease, being overweight, and others. HIV​/AIDS and having experienced significant trauma can also qualify.

Adult Medicaid recipients including managed care and dual-eligible – Medicaid and Medicare. Receiving the majority of their care in Monroe County, and the following:
- significant mental illness or serious emotional disturbance
- HIV​/AIDS
- complex trauma, if under 21
- two or more chronic health conditions (i.e. asthma, diabetes, heart disease, mental health condition, substance use disorder, etc.

Can accommodate other languages as needed.

Must have two or more chronic medical or mental health conditions, or one of the singly qualifying conditions. Must be enrolled in Medicaid. Children's Health Home: Birth to age eighteen. Adult Health Home: Age 18 or older.

Individuals with a developmental disability and who have been approved for eligibility through OPWDD. In-home respite open to all ages. Individual must live with family member. Respite program for medically frail primarily for folks who have, or are close to, aging out of school services.
