Must be 60 and over. Home-delivered meals are provided to those who are unable to prepare their own meals due to physical and​/or cognitive impairments.

Must be Medicaid eligible​/active Medicaid recipient and have: 2 chronic health conditions (asthma, diabetes, heart disease, COPD, obesity, substance abuse disorder, etc.) OR A single qualifying diagnosis, including serious mental illness (Bipolar Disorder, Schizophrenia, etc.), HIV​/AIDS, and significant behavioral, medical, or social risk conditions that can be addressed through care management.

4-H: ages 5-19 years. Call for information.

Medical care must be over a 2-hour drive to be eligible for free air transportation. The patient must also be ambulatory and medically stable as well as be able to walk and climb a few stairs to board a small aircraft.

Any service member who was active, deployed or wounded in the last 7 years is eligible for assistance, with proper documentation. Please see attachment in resource for eligibility criteria.
