Must reside in Wayne County. Early Head Start: Pregnant women and​/or with children birth to 3 years old. Head Start: Children from ages 3 to 5. Must meet income eligibility.

Senior citizens and individuals with disabilities who don't qualify to receive public assistance, Medicaid or EISEP.

Resident of Monroe county, at or below 200% of Federal poverty guidelines. Age 18 or older.

Individuals and families experiencing homelessness.

Resident of area served.

Must have two or more chronic medical or mental health conditions, or one of the singly qualifying conditions. Must be enrolled in Medicaid. Children's Health Home: Birth to age eighteen. Adult Health Home: Age 18 or older.

Medicaid recipients, including those who are already members of a Managed Care Organization (individuals who have both Medicaid and Medicare are also eligible). Those eligible need to meet one of the following conditions: Two or more chronic health conditions, such as asthma, diabetes, heart disease, mental health condition, or substance use disorder A significant mental illness A diagnosis of HIV ​/ AIDS
