Adult SPOA: Must have eligible diagnosis and need. Child SPOA: Must have eligible diagnosis and need. Assisted Outpatient Treatment (AOT): Must meet legal criteria set forth in Mental Hygiene Law 9.60.

18 years and older.

Participants with serious health issues and​/or other social​/emotional situations that are considered for eligibility can include but are not necessarily limited to the following: cancer, long term or chronic illness, Downs Syndrome, Autism, Asperger's, at-risk youth, Anxiety Disorder, depression, OCD, ADD​/ADHD, Post Traumatic Stress Disorder, Reactive Detachment Disorder, learning disabilities, documented social or emotional debilitation, life transitions​/family Issues, trauma, abuse, neglect and​/or life altering setbacks.

Must be income eligible to receive assistance through one of eight state programs: Medicaid, Home Relief, Supplemental Security Income, Aid to Families with Dependent Children, Home Energy Assistance Program, Food Stamps, Veterans Disability Pensions or Veterans Surviving Spouse Pensions.

Individuals with intellectual and developmental disabilities or other challenges.
