People of all ages may be able to pass life on through the gift of organ, eye and tissue donation. Medical conditions at the time of death determine what organs and tissues can be donated. Individuals should not rule themselves out when considering registering as an organ, eye and tissue donor. In New York State, persons 16 years of age or older may enroll in the New York State Donate Life Registry. See "Application Process" below to register to pass life on through the gift of organ donation.

Women who find theirselves unexpectedly pregnant, or parenting in difficult circumstances and needs some encouragement, accompaniment and support. Resident of area served.

Resident of area served, child​/youth under the age of 21, enrolled in Medicaid. The child​/youth must meet the NYS DOH eligibility. Significant behavioral, medical, or social risk factors which can be addressed through care management.

Pregnant women​/teens experiencing unplanned pregnancy.

Ages 18 or older. Individuals must carry a current mental health diagnosis and have needs that can be met through a mental health clinic.
