Physician recommendation and medical needs requiring long term care and​/or rehabilitation. An approved payor source is also required.

Spanish, Chinese, Italian

Must have an incurable illness with a prognosis of six months or less. Does Not wish to seek curative treatment. Must have a physician’s order for program services. Receive care in Ontario or Yates County.

Have prognosis of 3 months or less to live. Receiving hospice care at home but need additional assistance. Or, be in the hospital without the possibility of going home.
