• Matches individuals with a care manager to develop a plan to meet medical needs, including:

Care management.
Home Health Aides.
Medication Management.
Medical Equipment.
Physical therapy.
Speech therapy.
Respiratory therapy.

Eligibility: Individuals 18 and older who are eligible for nursing home level or care, can safely stay in their homes, need long-term care for 120 days or more, are Medicaid eligible, and live in area served.
