Eligibility: Over 5 years of age: specific physician referral. Behavioral Health Intensive Outpatient: Ages 18 years and older. Case management: ages 18 years and older.
Application Process: Call for information.
Fees: Medicaid. Medicare. Third party insurance. Sliding scale based on income. Private pay. Does not accept MVP.
