• Provides home health aides, licensed nurse practitioners, registered nurses, attendants, and personal care aides to work and oversee medical treatment in the home. Some of the services provided are:

Medication reminders.
Grooming and oral hygiene.
Bathing and dressing.
Mobility issues.
Meal preparation.
Laundry.
Housekeeping.
Errands.
Companionship.

• Provides a full range of services under the TBI/NHTD Waiver that includes the following:

Home and Community Support Staff.
Community Transitional Services.
Moving Assistance and Transportation.

• Offers non-medical transportation services.
• Provides care management through Health Homes of Upstate NY (HHUNY) and Children's Health Homes of Upstate NY (CHHUNY).
• Offers tuition free certification for for Personal Care Aides and Home Health Aides.

Fees: NHTD and TBI Medicaid Waivers, CDPAP, MLTCs, MCOs, long term care insurance, and private pay. We do not offer sliding scale payments.
