Provides case/care management to children and youth that are experiencing significant behavioral, medical, or social risk factors.

Eligibility: Resident of area served, child/youth under the age of 21, enrolled in Medicaid. The child/youth must meet the NYS DOH eligibility. Significant behavioral, medical, or social risk factors which can be addressed through care management.

Application Process: Complete online form, call for information.

Required Documentation: DOH forms as needed, call for information or visit website.

Fees: Through Medicaid.
