• Works in partnership with the Monroe County Reentry Task Force to provide case management that works one-on-one with men and women released for NYS prisons to ensure their stabilization needs are met. Supports include housing, employment and connections to treatment and other social supports.

Eligibility: Individuals recently released from a NYS DOCCS facility.

Application Process: Must be referred by parole officer. Call for an appointment.

Required Documentation: None.

Fees: No fees for service.
