• Provides parents who have given birth and pregnant people the support they need to be physically healthy, emotionally healthy, and self-sufficient. Services include:

On-site mental health counseling
Pregnancy education, financial, parenting, postpartum, and life skills classes.
Job training
Community Health Workers
Care management

Eligibility: Parents who have given birth and pregnant people, 18 years of age or older parenting a child under the age of 4 years. Resident of area served.

Application Process: Referral for enrollment in the program is required. Can be referred by a provider or self-referred.

Required Documentation: Insurance card and ID.

Fees: Insurance accepted or self-pay.

Coverage Note: Monroe County.
