• Coordinates care and communication among providers and offers support during transitions of care. Services include:
- Health promotion.
- Patient and family support.
- Referrals to community and social support services.
Eligibility: Age 18 and older. Must have chronic health conditions (physical or behavioral health). Must be eligible for Medicaid.
