• Provides care management and support services including:
- Meet your personal health goals and outcomes.
- Coordinate transportation to medical visits.
- Understand your medical needs, even if they are complex, and connect you to the care you need.
- Provide insurance assistance (ACA Marketplace, ADAP, private insurance, Medicaid and Medicaid spend-down, Medicare Part D).
- Attend medical appointments and pharmacy visits for medication follow-up.
- Find primary care and other kinds of medical care.
- Coordinate referrals and follow through for mental health and substance abuse.
- Explore other services, like nutrition education, day health, our food pantry, and more.
- Housing assistance and planning.
- LGBTQ-focused Care Management services

Fees: Case management: Medicaid billed or no charge. Other services: no fee for service.
