Children's Health Home Care Management
thru: Catholic Charities Steuben/Livingston
• Provides case management services to high needs children and their families.
Eligibility: Children from birth to 21 years, must have two or more chronic conditions, or Significant Emotional Disturbance, or Complex trauma, or HIV/AIDS. Must be enrolled in Medicaid.
Application Process: Call or visit website for referral form.
Required Documentation: Call for information.
Fees: No fee for services.
