• Provides in-home support services for adults with complex needs to reside in their own independent apartments.

Go into home to provide services.
Go with client into community to provide life-skills.
Liaison with other health providers including:
- Peer bridgers.
- Community living coaches.
- Financial case manager.
Linkage to community education.

Eligibility: Resident of supported housing reinvestment beds (RIV). Age 18 or older.

Application Process: RIV: Referral through SPOA. Non RIV: call for information.

Required Documentation: Completed application.

Fees: No fee for service.

Coverage Note: Monroe, Livingston, Wayne, Wyoming, Genesee and Orleans County.
