Eligibility: Age 6 or older: Specific physician referral. Behavioral Health Intensive Outpatient: Ages 18 years and older. Case management: Ages 18 years and older.

Fees: Medicaid. Medicare. Third party insurance. Sliding scale based on income. Private pay. Does not accept MVP.
