Eligibility: Varies with program. Hospice care: must be terminally ill with six months or less to live. Hospice care provided in Monroe, Wayne, and Seneca Counties.

Application Process: Referrals through physician for home nursing care evaluation. Through agencies or self-referral for homemaker-home health aide services.

Required Documentation: Call for information.

Fees: Third party insurance. Medicare. Medicaid. Private pay.

Coverage Note: Cayuga, Monroe, Livingston, Ontario, Seneca, Schuyler, Yates and Wayne Counties. Hospice is provided in Monroe, Wayne and Seneca Counties.
