• Assists individuals with:
- Coordinating care through communication with other providers.
- Advocating on their behalf to receive care.
- Navigating the healthcare system.
- Connecting to mental health and substance abuse providers.
- Connecting to needed medications.
- Linking to resources for finding affordable housing, benefits, food, clothing, childcare, transportation.
- Learning more about activities to improve health.
- Identify goals and ensure all providers involved in care understand the goals and care plan.

Eligibility: Must be Medicaid eligible/active Medicaid recipient and have: 2 chronic health conditions (asthma, diabetes, heart disease, COPD, obesity, substance abuse disorder, etc.) OR A single qualifying diagnosis, including serious mental illness (Bipolar Disorder, Schizophrenia, etc.), HIV/AIDS, and significant behavioral, medical, or social risk conditions that can be addressed through care management.

Required Documentation: Medicaid number. Completed Consent to Disclose Health Information Form must accompany the referral.
Fees: No fee for service for Medicaid recipients.
Coverage Note: Monroe, Wayne, Ontario, Livingston, Cayuga, Seneca, Steuben, Chemung, Schuyler, Allegany, Yates, Tompkins, Cortland, Erie, Wyoming, Niagara, Broome counties.
