• Provides supports to youth and families based on family preference and Level of Care determination. These include:

Caregiver Family Advocacy and Support Services
Planned Respite an Crisis Respite

Eligibility: Birth to twenty one. Must be a Medicaid, Medicaid Managed Care, or Medicaid Family of One recipient. Must meet Level of Care requirements.
