• Offers nursing support, advocacy and assistance coordinating doctor appointments, and help with understanding medical care plans and documents.
• Accompanies patients to medical appointments, coordinates transportation for appointments and medical treatments.
• Provides assistance with filling prescriptions and ensure linkages are made with other supportive community-based services.
