Service Coordination
• Coordinates all the community services to support individuals to live as independently as possible, including help with:
- Transitioning from a nursing home to their own home.
- Securing housing and financial benefits.
- Linking to community services such as transportation, nursing & aide services, health and medical specialists, etc.
Eligibility: Must be 18 years and older and show proof of brain injury or chronic health/medical issues.
Application Process: Call for an appointment.
Required Documentation: Documentation of Disability from a Physician/Specialist; Medicaid/Managed Care Certification.
Fees: Based on Medicaid/Managed Care Fees Private Pay Offered.
Coverage Note: Ontario, Wayne, Seneca, Yates, Monroe and Livingston Counties.

Bus Service:
RTS and Medical Taxi's and Cab Providers transport to our day program from Monroe, Wayne, Livingston, Ontario, Yates and Seneca Counties.
Disabilities Access:
