Service Coordination

thru: Bridges For Brain Injury, Inc

Coordinates all the community services to support individuals to live as independently as possible, including help with: transitioning from a nursing home to their own home, securing housing and financial benefits, and linking to community services such as transportation, nursing and aide services, health and medical specialist, etc.

Eligibility: 18 years and older and show proof of brain injury or chronic health/medical issues.

Application Process: Call for an appointment.

Required Documentation: Documentation of Disability from a Physician/Specialist; Medicaid/Managed Care Certification.

Fees: Medicaid/Managed Care Fees, Private Pay Offered.

Coverage Note: Ontario, Wayne, Seneca, Yates, Monroe and Livingston Counties.

Cultural Competency: Cultural competency, diversity, equity, and inclusion training for staff
