Service Coordination
thru: Bridges For Brain Injury, Inc
• Coordinates all the community services to support individuals to live as independently as possible, including help with: transitioning from a nursing home to their own home, securing housing and financial benefits, and linking to community services such as transportation, nursing and aide services, health and medical specialist, etc.
Cultural Competency
Eligibility: 18 years and older and show proof of brain injury or chronic health/medical issues.
Application Process: Call for an appointment.
Required Documentation: Documentation of Disability from a Physician/Specialist; Medicaid/Managed Care Certification.
Fees: Medicaid/Managed Care Fees, Private Pay Offered.
Coverage Note: Ontario, Wayne, Seneca, Yates, Monroe and Livingston Counties.
Cultural Competency: Cultural competency, diversity, equity, and inclusion training for staff
