• Offers access to support programs available through the New York State Department of Health providing Children's Health care management addressing chronic medical, mental health, substance abuse, behavioral health and developmental needs. Also to allow people with chronic conditions with or without developmental disabilities who need aid or nursing home care to hire and train care attendants through Consumer Directed Personal Assistance (CDPAP) a Medicaid service.
Eligibility: NYS resident, ages 5-30 who qualify for Medicaid and elderly with a disability, or requires assistance with activities of daily living, or needs skilled care. Must be medically stable and capable of overseeing their own care or have a designated representative.
