• Offers the Expanded In-Home Services for the Elderly Program that provides:

Cost-sharing home care for functionally impaired elderly.
In-home assessment.
Case management.
Subsidized homemaker/personal care aide services.
Subsidized housekeeping/chore services.
Subsidized adult day care and other limited support services.
Rides to the polls for 2024 election for current clients only.

Cultural Competency

Eligibility: Ages 60 years and older.

Application Process: Referrals from agencies: Peer Place, Eldersource and other agencies.

Required Documentation: Call for information.

Fees: Sliding scale based on income.
