• Provides case management services to help older adults and care givers of older adults.

In-home geriatric assessment; assesses areas such as social health, mental health (depression & anxiety), physical health, financial and safety
Personalized information and guidance about in home support and long term care options. Provides referrals as needed
Assessing and helping apply for public benefits
Assistance with decisions about housing options such as assisted living, adult homes, and nursing homes
Working with care givers on guidance for long term and respite options for loved ones

Eligibility: Older adults 60 and over, caregivers of any age of an older adult over 60.
