Managed Long Term Care Program

• Coordinates access to health, social, and long-term care services for people who are chronically ill or disabled and wish to stay in their homes, including:
- In-home nursing.
- In-home therapies.
- Home health aides.
- Personal care services.
- Adult day health care.
- Private duty nursing.
- Consumer-directed personal assistance.
Eligibility: Residents of area served, ages 18 and older who are Medicaid eligible. Must be in need for long-term care services of 120-days or more per year.

Coverage Note: Allegany, Broome, Cattaraugus, Cayuga, Chautauqua, Chemung, Chenango, Cortland, Delaware, Erie, Genesee, Herkimer, Livingston, Madison, Monroe, Niagara, Oneida, Onondaga, Ontario, Orleans, Oswego, Otsego, Schuyler, Seneca, Steuben, Tioga, Tompkins, Wayne, Wyoming, and Yates Counties.
