Ages 6-21, receiving Medicaid, with a chronic medical diagnosis.

CHILDREN: Birth to age twenty-one, active Medicaid., two or more chronic conditions. ADULT: Age 21 and older. Medicaid recipient.Two or more chronic conditions. Chronic conditions may include mental health, substance use disorder, asthma, diabetes, heart disease, being overweight, and others. HIV​/AIDS and having experienced significant trauma can also qualify.

Must have Medicaid coverage, age 18 or older, medically​/clinically qualify according to the Uniform Assessment System and reside in a community setting (or have an interest in relocating from SNF to community setting) in Monroe, Wayne, Seneca, Yates, Ontario, Livingston, and Genesee Counties.
