Birth to five years old, as well as school-age children.

Residents of Livingston County and surrounding areas.
Ages 18 years and older. Service provided per physician's orders. RN and social worker interviews on site or in home.

Livingston County resident receives priority placement. Medical substantiation of need. PRI screen. Signed admissions agreement.

Determined through the Office of People with Developmental Disabilities, must have an established diagnosis of a developmental disability (Autism, Cerebral Palsy, Epilepsy, Intellectual Disability, Traumatic Brain Injury, other Neurological Impairment) must exist prior to age 22, with an expected continuation of the condition. School Program: ages 5-21 years. Adult Training Program: ages 21+ years, no age cap and are Medicaid eligible. Community Habilitation: Medicaid eligible and by 23 years have one documented developmental disability.
