Must have Medicaid coverage, age 18 or older, medically/clinically qualify according to the Uniform Assessment System and reside in a community setting (or have an interest in relocating from SNF to community setting) in Monroe, Wayne, Seneca, Yates, Ontario, Livingston, and Genesee Counties.
