Residents of area served, ages 18 and older who are Medicaid eligible. Must be in need for long-term care services of 120-days or more per year.

Pregnant women​/teens experiencing unplanned pregnancy.

Medicaid recipients, including those who are already members of a Managed Care Organization (individuals who have both Medicaid and Medicare are also eligible). Those eligible need to meet one of the following conditions: Two or more chronic health conditions, such as asthma, diabetes, heart disease, mental health condition, or substance use disorder A significant mental illness A diagnosis of HIV ​/ AIDS

Residents of area served. Elderly and​/or individuals with traumatic brain injury.

62 years or older, require 24-hour supervision and must be ambulatory.

Must have two or more chronic medical or mental health conditions, or one of the singly qualifying conditions. Must be enrolled in Medicaid. Children's Health Home: Birth to age eighteen. Adult Health Home: Age 18 or older.

Individuals 18 and older who are eligible for nursing home level or care, can safely stay in their homes, need long-term care for 120 days or more, are Medicaid eligible, and live in area served.
