Eligibility: Developmental disability, autism, cerebral palsy, epilepsy and other neurological impairments which constitute a lifelong mental or physical impairment that becomes apparent prior to age 22 years, is expected to continue indefinitely and constitutes a substantial handicap to a person's ability to function normally in society.

Application Process: Call for assistance.

Fees: Sliding scale. Medicaid.

Coverage Note: Cayuga, Chemung, Cortland, Herkimer, Lewis, Livingston, Madison, Monroe, Oneida, Onondaga, Ontario, Oswego, Schuyler, Seneca, Steuben, Wayne, Wyoming, Yates counties.
