• Coordinates care and communication among providers and offers support during transitions of care. Services include health promotion, patient and family support, and referrals to community and social support services.
Eligibility: Resident of area served. Age 18 and older. Must have chronic health conditions (physical or behavioral health). Must be eligible for Medicaid.
